US VISA MEDICAL EXAMINATIONS

Times of examinations:  Weekdays mornings and afternoons and Saturday mornings

                                     Monday to Friday 8.30 am to 6.00 pm, or

                                     Saturday 8.30 am to 12.30 pm      

Time required:  1 to 1½ hours

Please bring the following with you:

  • The forms provided by the US Embassy or printed out from the Internet
  • A valid passport (not copy)
  • Glasses or contact lenses, if worn
  • One passport size photograph
  • Immunisation record, if available

Fees (including tax):

               

     Age 15 and over…….            ¥26,250

     Age 14 and under…...           ¥14,700

Immunisations (vaccinations):

The US immigration regulations require applicants to be immunised against certain infectious diseases, according to age.  If you bring a written record of immunisations or the results of laboratory tests for immunity, it may be possible to avoid having certain immunisations.  Blood tests, at additional cost, can also be done at to check for immunity.  If these tests are performed the documents will not be ready until the results are to hand, which may take up to one week.

The maximum cost for immunisations is ¥31,250 (plus tax) for an adult aged 20 to 49 years; in many cases it will be less.  These charges are additional to those for the examination.

Only vaccines normally available in Japan and approved by the Japanese Ministry of Health are used.   

 

Results:

  • Normal service.  If all immunisation requirements are fulfilled on the day of the examination, the documents will be available for collection three working days later.

 

  • Expedited service.  For an additional ¥8,400 (including tax) the documents will be ready for collection approximately two hours after the appointment time.   
  • Please note: if any abnormalities are found in the blood tests, the documentation may not be ready within the times stated above.  
  • Delivery service.  On request, the completed documents and X-ray will be sent by delivery service (‘Takyubin’), at the applicant’s risk and cost, thus saving a further visit to the Clinic.
